บทคัดย่อ/คำอธิบาย :

The measurement of pulmonary artery and pulmonary capillary wedge pressures is generally accepted as a reliable method in assessment of cardiac function and evaluation hemodynamic alteration in patient with heart disease. Traditionally, patients are positioned flat and supine for these measurements. The transducer is placed at the phlebostatic axis. These pressure measurements usually result in frequent repositions and may interfere with patients sleep pattern. Nurses may be tempted to leave the patient in a supine position throughtout hemodynamic monitoring. Unfortunately, long-term supine position sometime leads to circulatory and respiratory comlications. The purpose of this study was to study the effect of lying diffent positions on pulmonary artery and pulmonary capillary wedge pressures. The sample composed of 20 patients whose aged range were between 30 to 70 years. They all were the patients in medical cardiac care unit and surgical cardiac care unit of Siriraj Hospital. Each patient recieved 3 methods of measurement altermately. The first method was the measurement of pulmonary artery and pulmonary capillary wedge pressured in supine position while the transducer was placed at phlebostalic axis and right atrial level. The second was left lateral lying position head elevated 45 degree and the third was right lateral lying position head elevated 45 degree. In the second and third method, the measurements were done while the transducer was placed at previous phlebostatic axis and at the ritht atrial level respectively. Each method took approximately 7-16 minurwa. Data were analysed by analysis of variance of Split-plot randomized block desingn and Duncans New Multiple Range Test. The result were concluded as the followings. 1. There were statistically significant diffences in systolic, diastolic and mean pulmonary artery and pulmonary capillary wedge pressures among these three positions. (p<.01) 2. There were statistically significant differences in diastolic and mean pulmonary artery and pulmonary capillary wedge pressures between the two reference axis (phlebostatic axis and right atrial level. (p<.01) But there was no statistically significant difference in systolic pulmonary artery. 3. There were statistically singnificant differences in the interaction between position and transducers plece on systolic, diastolic and mean pulmonary artery and pulmonary capillary wedge pressures. (p<.01) From the result of this study it is concluded that the pulmonary artery and pulmonary capillary wedge pressures can be occurately obtained in lateral lying positions head elevated 45 degree by placing the transducer at the right atrial level.
